Chronic conditions: the care that happens in between
A chronic condition is not treated once and then ticked off. It needs someone keeping an eye on it over the years – and that is exactly what a family practice is for.
We provide long-term care for the common chronic conditions, including:
- Diabetes
- High blood pressure and cardiovascular disease
- Asthma and COPD
- Thyroid conditions
Much of the work-up happens here, without sending you elsewhere: the laboratory returns blood values during the same appointment, spirometry measures lung function in asthma and COPD, and 24-hour blood pressure measurement shows how your blood pressure behaves across a whole day – something a single reading at the practice cannot do.
